Hospital Emergency Incident Command System (HICS) 2008

The Hospital Emergency Incident Command System (HICS) was implemented at Marietta Memorial Hospital during 2004. HICS is a standard, hospital-wide response to disasters of all types. When the Nursing Supervisor, or the Administrator-on-Call, makes the decision to call a “Code Yellow”, the HICS system goes into effect. The CEO or Administrator-on-Call becomes the “Incident Commander” and immediately opens the Incident Command Center (located in the Administrative Boardroom). The Incident Command Center is stocked with radios, position descriptions, organization charts, color-coded vests, MMH Emergency Management manuals, Washington County manuals, and all other items necessary for the implementation of the HICS system.

 

Once the Incident Commander opens the Incident Command Center, he/she will assign positions in the HICS system to staff members. The staff members who will be assigned positions will follow instructions on a Job Action Sheet to manage the disaster. Not every disaster requires the assignment of all HICS positions, but the Incident Commander makes the decision of which positions are assigned.

 

What do you do during a “Code Yellow?” The most important thing you can do during a Code Yellow is to report to your work or designated area immediately. If you would be needed to fill a HICS position, you will be called to the Incident Command Center. Do NOT report to the Incident Command Center, unless called to do so. All

attempts to maintain normal operations during a Code Yellow are desired. Our number one priority is staff safety, patient care & patient safety. 

The following pages include the various codes you may encounter in the hospital emergency management system. Several codes were changed in 2004 and a couple of new ones in 2007. The changes were brought about to make a more standardized system used in many hospitals throughout the United States.

Fire

 

Fire, smoke, or smell of something burning.

Rescue those in immediate danger (if   safe to do so). Pull Fire Alarm then Call 1444. Contain the fire (close         doors). Extinguish the fire if safe to do.

Use an extinguisher to put out the fire. Pull the pin, aim, squeeze the handle. Sweep from side to side.

Identify any personnel unaccounted for.  Evacuate if appropriate.

CODE YELLOW

Disaster 

A disaster that could be natural or man-made, internal or external.

All employees report to their work or    designated area immediately.

Specific tasks will be assigned through the Labor Pool Coordinator

Relay information as needed.

Tornado sighted in area.

Personnel will remain in hospital or  return inside.

Remove objects from rooms.

Continue process until "All Clear" is paged.

CODE BLACK

Bomb Threat

 

Notification of bomb on campus, usually by an outside caller.

Obtain as much information as possible: Where is bomb, when it  will go off, what does it look like, why was it placed, etc.

Report all information to your Supervisor and Security.

Search area for bomb.  Do not touch if found. Report anything suspicious. Fill out bomb threat form in Emergency Prep. Man.

Hazardous Material, Spill, or Release

Incident Spill

Small spill presenting NO hazard.

Emergency Spill: Any spill, which may present a hazard.

 

 

Contaminated patients coming to MMH.

Trained user cleans up spill with PPE.

 Isolate the spill area (evacuate). Deny   entry to others.  Notify your Supervisor.  Dial 1444.

 

The hospital decontamination team will be activated.

Appropriately dispose of materials.

  

 

 

Maintenance & Decon. Team set up decon. site. Call rosters for decon. team activated. Prepare for patients.

CODE SILVER

Hostage 

An individual is being held against their will by an armed perpetrator.

Clear the area to avoid others from   becoming a hostage. Dial  1444.

Report all pertinent information to Supervisor and others in charge of response.

Immediately report information on any suspect to Security.

CODE ADAM

Infant Kidnap

 

An infant is missing or is known to be kidnapped.

Go to closest exit and watch for a person with an infant or with a package that could hold an infant,  that is not     being escorted out by an Orderly or      Student  Nurse.  Dial 1444.

Ask to verify infant identity (wrist name tag) or see contents of package. Get clear description of adult and note direction of travel.

Immediately report information on any suspect to Security.

CODE WHITE

Utility Failure 

Loss of one of the following utilities:  electric, telephone, steam, gas, water.

Refer to the System Failure &  Basic    Response Sheet.

Follow  procedures of user on the Response Sheet.

Continue actions until "All Clear" is paged.

CODE IT

Notification of severe weather with electrical surges or lightening.

All employees are to turn off and unplug all personal computers,              terminals, and printers.

Assist as directed.

Code IT All Clear will be paged when threat of severe weather has passed.

CODE ZERO

Evacuation 

Remaining in area may be hazardous to life, health, or safety. EVACUATION.

Notify all in area of need to evacuate.   Evacuate ambulatory wheelchair, and   bed ridden. Take records if safety permits.

Report to designated assembly area and account for all who were in previous area.

Report evacuation status to Central Command Post.

CODE HOT LAB

Radioactive Incident

Isolate the spill area (evacuate).  Deny entry to others.  Notify Supervisor.

Notify Nuclear Medicine who will coordinate response.

Compile information as requested.

CODE EARTHQUAKE

Significant shaking of the building.

Duck and Cover. Protect self by taking steps to improve safety. Get under a table, move away from objects likely to fall, protect head.

Triage yourself, survey damage, take appropriate defensive action.

Report problems to Department Manager or Supervisor.
